- All eligible Employees of a member Company under age 65. Amount is 66.7% rounded to the nearest dollar, up to a maximum benefit of $5,000 per month. Waiting period is 120 days.
- Dental Care [Employees and dependents]
- No deductible 80% reimbursement for basic services and 50% for Dentures, crowns and bridgework $1500 annual maximum per person.
For dependent children, 50% reimbursement of Orthodontics to a lifetime maximum of $1500.
